Photograph Grant of Use Form
-- to be completed by the photographer (must be completed for any
submitted photographs to be published on this website). A printable
copy of this form is located HERE
and a fillable copy of the form is located HERE.
- SCA
Model Release Form
-- to be completed by the subject(s) of the photograph if the subject's
face is identifiable or in portrait-style photography or photography
taken in a private space or not a public venue. This form is not needed
when the photograph is taken in a public place or area where privacy is
not expected, such as SCA contests, classes, court, merchant area, and
the like. A printable copy of this form is located HERE
